How to receive Home Care Service:

1. Obtain a referral from your physician. 

2. Inland Wellness Health Care, Inc. will then obtain prior authorization or eligibility from your insurance. 

3. Once authorization or eligibility is obtained Inland Wellness Health Care, Inc. will coordinate care and/or services for you.  

    

Who Can Receive Home Care Services:

• Home-bound and unable to receive outpatient services 

• Sick or disabled 

• Chronically/Terminally ill 

• Convalescent 

• In need of skilled nursing services
